Privacy Policy

Resurgo Physical Therapy and Performance LLC ("Resurgo," "I," "me") operates this website. This policy explains what information I collect through it, why, and what happens to it.
What I collect
When you fill out the contact form on this site, I receive the information you enter: your name, email address, phone number if you provide it, your preferred day and time, your reason for reaching out, and anything you write in the comments field.
If you book a Discovery Call, that scheduling is handled by Calendly, which collects your name, email and selected time. Calendly's own privacy policy governs what they do with it.
Like most websites, this one also collects basic technical information automatically — pages visited, approximate location, browser type, and how you arrived. This is standard web analytics and it isn't tied to your name.
Please don't send health information through the contact form
The contact form is a normal web form. It is not a secure or encrypted channel, and it is not the right place for detailed medical information.
Tell me enough to know whether I can help — the body part, roughly what happened, what you're trying to get back to. Save the detail for the call, or for the intake paperwork, which runs through a secure system built for it.
What I do with it
I use it to respond to you, to schedule a Discovery Call, and to prepare for that conversation. That's all.
I do not sell your information. I do not share it with advertisers. I do not send marketing email to people who filled out a contact form.
If you become a patient, your clinical records are kept separately in a secure, HIPAA-compliant records system — not in email, and not on this website.
Who else touches it
- Squarespace hosts this site and processes form submissions
- Calendly handles Discovery Call scheduling
- Google provides email and analytics
Each has its own privacy terms. I've chosen services appropriate for a healthcare practice, but I don't control their policies.
How long I keep it
Contact form submissions are kept while there's an active conversation and for a reasonable period after. If you ask me to delete your inquiry, I will.
Your choices
You can ask me what I hold about you, ask me to correct it, or ask me to delete it. Email me and I'll take care of it.
A note on HIPAA
HIPAA protects health information held by a healthcare provider about their patients. Filling out a form on this website does not make you a patient, and this policy is not a HIPAA Notice of Privacy Practices.
If you become a patient, you'll receive that separate notice as part of your intake paperwork. It explains your rights over your health records in detail.
Children
This site isn't directed at children under 13 and I don't knowingly collect information from them. If your child is a prospective patient, a parent or guardian should make the contact.
Changes
If this policy changes, the date at the top changes with it.
